Homelessness & Assertive Outreach
In Cardiff, we work as part of the Council’s Homeless Multidisciplinary Team (MDT) to deliver the following support:
- Rapid Access Prescribing
- Substance Use Outreach Support
- Counselling
Alongside this, our Peer Navigator team offers targeted outreach, building trusting relationships to help individuals understand and engage with the range of support services available to them.
Rapid Access Prescribing Service (RAPS)
Cardiff RAPS is a specialist service for people experiencing entrenched homelessness, delivered on behalf of the Cardiff Council Homeless Multidisciplinary Team (MDT).
The service provides rapid access to substitute opiate prescribing, including the option of Buvidal (long‑acting buprenorphine injection). Clinical provision also includes harm reduction interventions such as take‑home Naloxone, sexual health screening, Blood Borne Virus testing, and referral into specialist support where required.
Referrals are made via the Cardiff Council MDT. Following referral, individuals are assessed by a multidisciplinary team comprising a case worker, a nurse, and a doctor before a prescription is initiated.
Treatment is personalised through individual recovery‑focused care planning, supported by regular clinical assessments and reviews.
Once an individual is stabilised on their prescription, the aim is to transition their ongoing care into mainstream services.
